Text Box: Elements of the case management process include:

ü	Case finding- the early identification of the TB patient to ensure that public health reporting regulations are upheld and TB control activities initiated
ü	Assessment-gathering data from many sources to begin TB treatment and care.  Patients who are diagnosed during a hospitalization will require discharge planning.  Ongoing assessments take place regularly in the clinic setting or private physicians’ office.
ü	Problem identification-assess  and document existing and/or potential health problems
ü	Plan development-establish and ensure that all required components are included
ü	Implementation-monitor the patient’s response to TB treatment, interventions, and adherence
ü	Evaluation-desired outcomes must be evaluated to make sure that treatment goals are achieved
ü	Documentation-proper documentation enhances the continuity of care over the course of treatment

Case management is directed towards assuring that patients complete TB treatment within appropriate time frames and with minimal interruption in lifestyle and work. Each patient receives TB care and treatment according to published standards of care.  Transmission of TB within the community is prevented through effective contact investigations and control activities.

Case management is a system of healthcare delivery in which an individualized treatment plan for the patient is developed by a multidisciplinary team to achieve established patient care outcomes.
